Low Back Pain Is Not One Single Problem

What biomechanics can add to a careful, person-centered examination.

Clinical illustration of lumbar movement assessment

Low back pain is common, but the label alone does not explain why one person hurts while bending, another while standing, and another after prolonged sitting. A useful evaluation starts by looking for serious or specific causes, then connects symptoms with function, movement tolerance, strength, work demands, sleep, stress and the patient’s goals.

What a biomechanical examination may add

Biomechanics is not simply a search for a “bad posture” or one joint that is “out.” It examines how forces, movement strategies and capacity interact. Depending on the presentation, the clinician may assess lumbar and hip motion, neurologic findings, repeated movements, lifting strategy, balance, trunk control and the tasks that matter most to the patient.

The key question is not only “Where does it hurt?” It is also: what makes it better or worse, what can the person currently tolerate, and which modifiable factors may support recovery?

Why treatment should be individualized

Clinical practice guidelines support active care for many people with persistent low back pain. Exercise selection can include general conditioning, trunk strengthening and endurance, movement-control exercise, aerobic activity or other approaches matched to the person. Education should encourage safe activity and reduce unnecessary fear—not promise that one technique works for everyone.

Manual therapy may be one part of care when appropriate, but it should sit inside a broader plan that tracks function and builds the person’s ability to move, work and participate in life.

What progress can look like

  • Greater tolerance for sitting, walking, lifting or sleep positions.
  • Improved confidence and reduced fear during meaningful movement.
  • Better strength, endurance or movement options.
  • A clear self-management plan for future fluctuations.
