Symptoms after a collision can involve the neck, head, back, shoulder girdle or nervous system, and they do not always follow the same timeline. A careful evaluation begins with the mechanism of injury, symptom onset, medical history, current function and screening for findings that require urgent or specialized care.

Safety comes first

New or worsening weakness, significant numbness, loss of coordination, severe or unusual headache, repeated vomiting, loss of consciousness, difficulty speaking, chest pain, breathing difficulty or other concerning symptoms require prompt medical attention. Clinical evaluation is not a substitute for emergency care when red flags are present.

What the examination may include

• Neck and upper-quarter motion, symptom response and functional tolerance.
• Neurologic screening such as strength, sensation and reflexes when indicated.
• Assessment for headache, dizziness, balance or concussion-related concerns.
• Shoulder, thoracic and lumbar findings based on the individual history.
• Baseline measures that can be repeated to monitor change.

Why recovery should be monitored

Research shows more than one recovery pathway after whiplash-associated injury. Higher initial pain and disability, psychological distress and other factors may be associated with slower recovery. This does not determine an individual outcome; it helps the clinician identify who may benefit from closer follow-up and a more coordinated plan.

When serious injury has been excluded, advice, appropriate movement and progressive activity are commonly important. Care should be matched to irritability and function, with referrals when symptoms fall outside the clinician’s scope.
